MFP Educational and Informational Materials

The University of Illinois at Chicago is providing educational modules and handouts as a resource for MFP transition coordinators to use to enhance the safety and success of community transitions. On this page you will also find Informational Notices specific to the populations served in Pathways/MFP

Some topics have PowerPoints that have been developed with transition coordinators as the target audience; these modules all include learning objectives, application to example case studies, and questions for reflection and comprehension. The goal is to increase the readers basic knowledge around that topic area. Other materials available are self-management guides, web links, simple assessments and basic informational handouts for participants and caregivers.

These materials should not take the place of professional medical advice. There are several mini-assessments available. The results of such assessments should be shared with the primary care physician so that appropriate treatment adjustments can be made.
